   I’m thinking a really foreshortened rear view of a person’s leg, as they kneel on something white…with the top of the “thumb” as an upturned heel.

   I think it’s thumbing a cuff, as if adjusting a cuff link.

   By heel, do you mean the bottom of a foot?  I see that.  If it’s a thumb, it’s a girthy one.  Very…bottom heavy.
